A grant request form appears in the Foundation's Annual Report or may
be obtained by clicking on BAF Grant Request Form. A copy of the grant request form must be used as a cover
sheet for each proposal. All information specified on the grant request form must be submitted by the December 15 deadline.
Applications should be clear, concise and legible.
